Site groups: setting up, closing and reopening a CLIA site

Create a site group for each CLIA-licensed site with its address, licensure and director, and close it in place when the site stops testing.

A site group is one CLIA-licensed site with its own street address. Everything in Legion Diagnostics is filed under a site group, and people see only the site groups they belong to, apart from the Primary Site Administrator, who sees them all.

Creating a site group

Click "New site group" and work through four steps:

  1. On "Site", enter the name, street address, time zone, an optional description, and members.
  2. On "Licensure", enter the CLIA certificate type and number, issuing body, jurisdiction and dates, plus any CAP accreditation or state license. Matching public CMS records are suggested.
  3. On "Designations", choose the Lab Director and CLIA License Holder. Designating someone never changes their role.
  4. On "Review", check your entries.

The site's test complexity and its other CLIA positions are recorded later in Site licensure & designations.

Members

Add members from the site group's "Actions" menu, and remove them from the member list. Site Managers can manage the members of their own site groups.

Director site visit policy

People who manage access control also see a "Director site visit policy" section on this page. It sets how often each site's lab director must be on site under your internal policy, which can be stricter than the CLIA 6 months but never looser, and keeps a history of every change with its reason. To give one site group its own cadence, select it, click "Actions", then "Visit policy…", choose the number of months or the organization's cadence, and give a reason. See Recording lab director site visits.

Closing and reopening a site

A site group's records never move to another site. When a site stops testing, close it rather than deleting anything. Closing asks for a reason and shows what will be affected. A closed site's records stay visible and read-only, its scheduled work pauses, and members can still view and sign off past work. Reopen it to pick up where it left off.

Archiving

Archive a site group only if it was created by mistake, and only while it is empty. One that holds records must be closed instead. You cannot close or archive the last remaining site group. Archiving and restoring each ask for a reason, which is recorded in the audit trail. Closing, reopening, archiving and restoring all need the "Close, reopen and archive site groups" permission.
